How to Create and Display Document Attributes

Every time I create a new documen, I'd like it to display custome attributes like these ...

• Created date/time.
• Source URL

I read the hlep manual and I can see that I can create custom document attributes. But how do I configure Siyuan to show these attributes every time I create a new document.
